Abdominal Aortic Aneurysm

Abstract
Recently, the incidence of abdominal aortic aneurysm is gradually increasing in Korea. This aneurysm is the among the most common and most dangerous diseases encountered by vascular surgeons. Rupture of the abdominal aortic aneurysm has long been
recognized as a lethal event. Thus, a more aggressive policy of early diagnosis and elective prosthetic graft replacement is today the treatment of choice for most abdominal aortic aneurysms and can be performed wit a mortality rate of less than

percent. We analyzed 23 patients with abdominal aortic aneurysms who were operated on at the Korea University hospitals between 1990 and 1993. The results were as follows; There were 20 male and 3 fmale patients. The mean age was 60.5 years.
Emergency
operations were performed for 4 patients with ruptured aneurysms and others were operated on electively. Endoaneurysmal graft implantations with 19 bifurcation and 2 straight graft were performed for 21 patients with infrarenal aortic aneurysms.
One
suprarenal saccular aneurysm was resected and repaired with patch graft via retroperitoneal route. One infected aneurysm was resected after axillobifemoral bypass surgery. There were 5 palients who had not been operated on at the time of the
diagnosis
for various reasons. among them, 2 patients with ruptured aneurysms expired. One free peritoneal and 4 retroperitoncally contained ruptures were included in our cases. Four patients were operated on emergently. There were 2 postoeperative deaths.
In conclusion, we should consider the possibility of abdominal aortic aneurysm as a cause of unexplained abdominal pain in aged patients and screen with abdominal sonogram. We think early diagnosis and elective operation regardless of the size
are
crucial for the better outcome in the management of the patient with abdominal aortic aneurysm.
